Dying pyernees

Just wondering if anyone had any input on this, my cousin had one that got a blood transfusion due to an injury she was unaware of and the dog, which was not even barely a year old was actually getting better a couple days after blood transfusion and then they got home to find the dog dying and a foul smelling liquid was pouing out the nose like a water faucet and the dog passed away.  Any thoughts of what would have cause it?
