life expectancy

hello,

my german shepherd was fat and lively till october last year, when i left the country, and now, when i came for a visit to my parents, i had a shock: he's thin, he has a swelling near his butt, he seems a bit deaf, he falls a lot when he stands up after sitting for a while, walking like a drunk man... his ear problem and teeth problem are a few years old and nothing i tried helped, not to mention that my country has poor conditions for people, let alone good vets and hospitals for animals, so the only answers i got were 'ur dog is 85 in human years, it's normal' bla bla...

my question is, for those who benefit from all medical wonders, is my dog suffering from the lack of medical help (in which case i'll feel guilty for ever, even if it's not really my fault), or is he really old and such things happen cos of age ? is 12 a critical age for a german shepherd ? what's next in the process of getting old, what else shud i expect in the future ?
