Heart murmur

Chili (chihuahua) is not doing well. He is 13yrs old and has a heart murmur, always has. It has now progressed to a 5 on a scale of 1-6. He has recently developed a horrible honking cough and usual symptoms associated with this condition.

I love my vet and we are pro-active in treatment. He is now on cough suppressant and medication to relieve some of the fluids from building up around his heart.

I'm going around, la la la la thinking nothing of this. This is just what we need to do now for the Chilman. But the problem is, he is not getting better.

I think it was the post about sweet, sweet Bear that got me thinking... is this really serious? Can he die from this? Are we getting into that area :'(  :'(?

Anyone had any experience with heart murmurs as far as seriousness and downward progression? My vet hasn't said this is the beginning of the end as we are dealing with the problems at hand, but I didn't ask.
